I had worked with non profit organization in various fields such as Health, Education, Agriculture & Research. During this entire perioed, I had learned many skills like as project management, database management, capacity building, conflict management, team building, report writing. While I have developed strong coordination, management, presentation, documentation, reporting skills as well as strong liaison with government department.
KEY JOB RESPONSIBILITIES
1. Transport of sputum specimens from BMUs to nearest X-Pert testing sites and transport back the results within the tehsil/jurisdiction.
2. Ensure the safe and secure transportation of samples, following the established protocols
3. Visit all new registered patients at their homes within one month of treatment initiation, maintain list of verified address, and do contact tracing.
4. Ensuring initiation of TB Preventive Therapy as per advice of a registered medical physician to all eligible contacts of B patients after ruling out active TB.
5. Work closely with health facilities and community health workers to locate and retrieve TB patients who have been lost to follow-up (LTFU) / do not respond to telephone calls from DOTS facilitators.
6. Encourage and support LTFU patients to resume TB treatment.
[1/11, 12:01 PM] Waseem ITA: 7. Maintain accurate and up-to-date records of all TB-related activities, including sputum transportation, contact screening, and lost to follow-up retrieval that is being verifiable from HMIS/EMR
8. Maintain a map of the tehsil, detailing all health facilities in the area, both government organizations and NGOs which specifically carry out TB activities, including the staff responsible for these TB activities (name, position and location).
9. Participate in Health Screening Camps within your district, conducting verbal screenings to identify potential TB cases for further evaluation as well as ensure data entry into department's electronic screening portal.
10. Ensure timely and accurate reporting of data to the PTP Punjab.
11. Undertake any other program related activity as directed by PTP and/or DTO/CEO DHA
Provide services and understand the positive impact of services on “Target Population” (spouses, sexual partners and children of PWIDs).
Effectively implement the assigned project related activities as per the approved work plan and against set targets
Register spouses of Persons Who Inject Drugs (PWID) as per policy and keep a copy of their registration record and also coordinate with DEO on daily basis.
Conduct daily counseling sessions with spouses of PWIDs on issues including HIV, safer sexual practices, health & hygiene, role of family in clients care, and also motivate them to get tested for HIV.
Distribute syringes (wherever needed) and condoms among spouses of PWIDs as per project policy and maintain daily record and hand it over to the designated staff member
Refer spouses for medical services to the medical team
Refer spouses for HTC to COPC site.
Develop reports of daily activities
Follow the referral guidelines and ethical protocols while working in the field.
Coordinate with other staff members to have insight into the issues related to performance and regularly report to the Site Manager for guidance and technical support
Identify and highlight sensitive and important areas of concern regarding spouses of PWIDs so that those areas can be further worker out for research or service delivery.

Maintain a healthy and cooperative relation with the patients and their relatives
Educate the patients, relatives or other members on various information, rules and process
Refer clients to community or social service programs.
