Using Community Informants to Estimate Neonatal Mortality Rate (NMR) in KPK
This inception report summarises the proposed methodology and work plan for the study entitled “Using the Community Informant Based (Made-In and Made-For Methodology for Estimating the Neonatal Mortality rate (NMR) in district Nowshera, Khyber Pakhtunkhwa.
Using Community Informants to Estimate MMR in 2 Districts of KPK
Through its Reproductive Maternal and Neonatal Child Health (RMNCH) project, GIZ Pakistan is assisting the Government of Pakistan in strengthening the capacities of the health system to ensure effective, efficient, client-oriented, and affordable reproductive, maternal, newborn and adolescent health care provision.
GIZ approaches senior national and provincial government officials indicated their interest to the Population Council in seeing the approach applied up in two districts of KP, Haripur and Nowshera, to assess the feasibility of estimating the MMR using community-based informant networks.
The primary objectives of the study were:
• To assess the feasibility of applying the MADE-IN/MADE-FOR methodology in two districts of KP
• To obtain baseline data on maternal deaths in the two project districts
• To build capacity in KP to obtain maternal mortality estimates through community-based systems involving communities, the local government and the KP Health Department in a sustainable manner.
Secondary objectives of the study were:
• To identify the sustainable networks available within rural and urban communities that can act as key informants to provide information on maternal deaths in KP
• To determine differential characteristics of the maternal deaths (economic status, urban and rural)
• To assess the mechanisms that can be employed at the community level to determine the causes and circumstances of deaths.
Landscaping Analysis on Pre-Eclampsia and Eclampsia in Pakistan
The primary goals and objectives of this landscape analysis was to identify the policy and level of program activities; gaps and challenges around prevention and treatment of PE/E and use of MgSO4 at the provincial and district levels in Pakistan. The role of non-government organizations, development partners and others (including professional associations) was also reviewed to which extent they are involved in PE/E activities, the policy makers, and stakeholders’ understanding of PE/E and use of MgSO4 in the management of PE/E.
This report presents the findings of a landscape analysis of family planning (FP) in Pakistan by the Population Council in 2015 and 2016, with the support of the Bill and Melinda Gates Foundation.
This landscape analysis assesses both demand and supply side shortcomings in the FP market, and provides a comprehensive assessment of consumers, service provision, contraceptive supply, and policy and regulations to help identify opportunities for investment that could catalyze an escalation in contraceptive prevalence, enabling Pakistan to meet its FP2020 goals.
The five objectives of the study are to:
1. Comprehensively assess the coverage of FP services, constraints of different segments of consumers, the limitations of contraceptive choice, particularly out-of-pocket costs of contraceptive methods, and opportunities to expand access and choice in three Pakistan provinces in a market segmentation approach;
2. Identify, analyze, and address supply and demand barriers in the public and private sectors;
3. Assess the market for commodities and services for FP, especially suppliers’ perspectives;
4. Assess the policy landscape and scale of donor investments in FP; and
5. Suggest future strategies and areas of investment for FP in three provinces in Pakistan.
Taking Forward Advocacy & Media Campaign on Population and Development
The purpose of the project is to design and implement a national level campaign on population issues to generate sustained political will and commitment within the current and future governments for universal coverage of family planning services to balance the population growth and the social and economic development of the country.
The project aims to change current complacency around the population issue, to desensitize the issue through strong arguments for raising responsibility, to get a coalescence of opinion across various groups converging towards national responsibility, and ultimately generating political will and commitment within the current and future governments to ensure universal coverage of family planning services in Pakistan.
For achieving this purpose, a national-level campaign will be prepared and implemented that will include direct advocacy with national and provincial policy makers and utilizes mass, print and social media to heighten awareness and deepen understanding of the important linkages of population issues with family well being, poverty, health education and economic betterment and to move to suggest individual and collective actions for creating a balance between population numbers and resources available in the country and for families.
An Independent Assessment of LHWs in Sindh Province-Pakistan
This assessment by the Population Council, commissioned by John Snow, Inc. (JSI) and supported by the United States Agency for International Development (USAID), of the strengths, weaknesses, opportunities, and threats (SWOT) facing Sindh’s LHW program specifically examined 1) program, managerial, and operational issues inhibiting its more effective implementation; as well as 2) the Sindh government’s vision for the program, including increasing operational efficiencies and coordinating expansion in areas currently not covered; and 3) further strengthening of the program’s functional integration and coordination with other maternal and child health (MCH) programs, particularly the Maternal, Newborn and Child Health (MNCH) Programme, Nutrition Programme, and Expanded Programme of Immunization (EPI).
Using Community Informants to Estimate Maternal Mortality-PUNJAB
Planners in Pakistan require a method that can provide reliable sub-national estimates easily, cost-effectively and with greater regularity. In this regard, the Research and Advocacy Fund (RAF) offered support to the Government of Pakistan to assess the feasibility of testing a new community informant-based approach, “Made-In Made-For” (MIMF), for estimating the MMR at the community level. A pilot study was conducted by the Population Council to test the approach in Chakwal, a district of Punjab, with promising results. Subsequently, in view of the diversity within Pakistan, particularly across the four major provinces, and to gauge the potential and modalities of the approach at a broader level, senior national and provincial government officials indicated their interest in seeing the approach scaled up in at least one province, i.e., Punjab, to provide reliable provincial level estimates.
The primary objectives of the study were:
• To apply the community informants networks technique (Made-In Made-For) to obtain a provincially representative current estimate of MMR for Punjab and to identify the causes of deaths.
• To build capacity within Punjab to obtain district and provincial maternal mortality estimates routinely, on a sustainable basis.
Secondary Objectives of the study were
• To identify the networks available within rural and urban communities of Punjab that can act as key informants for routine information on maternal deaths;
• To determine differential characteristics of maternal deaths by economic, urban and rural, and geographical distribution so as to gain a better understanding of the predisposing factors contributing to women’s death during pregnancy, delivery and the postpartum period;
• To assess the mechanism that can be employed at the community level to determine the cause of deaths.
task-sharing between CHWs and community mid-level Professionals
We undertook this exploratory qualitative research in all four provinces of the country. The following two types of data collection were used:
1) Focus Group Discussions (FGDs) with CMWs and LHWs were conducted to identify the current mode of supervision, barriers to working together and other constraints faced by both types of providers in the provision of MNCH services. We wanted to better understand the perceptions of community based midwives (CMWs) and LHWs regarding each other’s roles and responsibilities and the level of existing working relationships between the two cadres.
2) Key informant interviews (KIIs) were conducted with provincial managers of LHW and MNCH Programs in the four provinces to ascertain how strong the working relationship and coordination between the programs is at all levels, as well as to identify any programmatic gaps and potential modalities for working synergistically.
After the study findings it was stated that:
"It can be concluded that there is weak coordination and communication between the LHW Program and MNCH program at all levels. This gap widens at the operational level where both the cadres (LHW and CMW) view each other as competitors and not as complementary tiers to achieve the goal of improvement in MCH services.
Family planning is not a priority for either cadre; with LHWs due to their heavy involvement in the polio program and in case of CMWs as they consider their main task is conducting deliveries.
Current supervisory mechanisms are weak. Providers and managers reported that the main responsibility for supervision of CMWs lies with the LHS but this mechanism does not function due to a variety of reasons which include problems with funding, transportation, and already overburdened supervisors."
Training Social Science Research Methodology in Public Sector Universities
As part of a quality assurance mechanism the Council carried out periodic assessments of the quality of teaching within the universities and medical institutions. One of the consistent recommendations received from the faculty was the need for improving their research capabilities so as to lead to an improvement in the quality of research being currently undertaken. In order to meet this expressed need the Population Council approached the World Health Organization to seek assistance in organizing a teaching course for trainers in research methodology. It was fortunate that the proposed activity was in sync with WHOs Special Programme for Research, Development and Research training - Grant to Support Group Learning Activities.
After the approval of the proposal, preparatory work was carried out that included development of the course curriculum, teaching materials, course brochure and admissions form. Resource persons were approached and their availability ascertained.
The Course was organized from June 9th-20th and was held at a local Hotel where most of the outstation participants were put up.
In order to address the regional need for boosting research capacity, and encourage South – South collaboration, we had invited two participants from Afghanistan. These were staff members from Kabul Medical University and Afghan Public Health Institute and selected by the Afghanistan Ministry of Public Health. Unfortunately one of the nominees could not attend the course due to consular reasons.
Countdown on Millennium Development Goals 4 & 5 in Punjab Province
A desk review was carried out by the Population Council staff that thoroughly investigated all relevant databases, reports, and other documents to provide a comprehensive assessment of the maternal neonatal and child health situation in the province of Punjab. Additionally, a brief analysis of the district and regional disparities in MDGs 4 and 5 and related family planning and fertility outcomes using the PDHS 2007, the PSLMS 2006 and the MICS data was carried out. We also looked at the relationships between fertility and maternal and infant mortality and how these vary by levels of development. This helped focus the contributions of family planning and fertility trends towards the achievement of reductions in maternal and child mortality in specific setting of Punjab and district level differentials. Since the district is the focus of implementation such information was useful in providing inputs into the background paper. The team also reviewed systemic issues which impede achieving better maternal and infant health service delivery outcomes focusing on human resources issues, financial budgetary resource allocation and priority setting mechanisms, including an analysis of what has worked and what has not, as well the lessons learnt. The assessment also included a review of reports of initiatives undertaken in the public and private sectors such as the government’s MNCH project and donor supported programs e.g. PAIMAN, FIGO Pilot, AMDD and Population Council’s SMART project. To complement the desk review and identify future strategic direction for achieving MDG 4 and 5, new information was collected through in-depth interviews with selected key informants. These interviews were held with both the provincial and district officials. At the provincial level, meetings and interviews were held with the Secretaries of Departments of Health and Population Welfare, Director Generals of both departments. officials of the MNCH and National program for FP and PHC.
From June 2007 to March 2012, FALAH worked to increase adoption of World Health Organization–recommended birth spacing practices according to the Healthy Timing and Spacing of Pregnancies guidelines (a minimum interval of 24 months from birth to next pregnancy). Because the views and opinions of religious leaders and scholars are critical in shaping public opinion, FALAH sought to educate and inform them on the need for more effective family planning interventions to help mitigate perceived religious opposition.
Efforts to increase religious and community leader support for birth spacing included consensus-building meetings with religious scholars to solicit their views and involvement in promoting birth spacing; coordinating television interviews with prominent stakeholders; and arranging visits to Islamic countries to observe advocacy programs with religious leaders and discuss how they can be applied in Pakistan.
The Impact
FALAH improved knowledge about the benefits of birth spacing; raised awareness of risks associated with closely-spaced pregnancies and pregnancies too early or late in a woman’s life; expanded knowledge of contraceptive options; and improved access to quality care in the public and private sectors. FALAH achieved these goals by building capacity among healthcare providers, encouraging the adoption of new methods (e.g., Emergency Contraceptive Pills, the Standard Days Method), and supporting the commercial sector in marketing and distribution of contraceptives.
FALAH increased contraceptive use by 8% in just four years. Unmet need for birth spacing fell from 14.2% to 10.8% over the same period. Through group meetings with trained educators and free consultations at mobile clinics, more than half a million people received family planning services. Messages raised awareness of birth spacing benefits and brought about attitudinal and behavior change. FALAH sensitized over 1,500 district-level religious leaders.

Pakistan Initiative for Mothers and Newborns (PAIMAN)
The USAID-funded Pakistan Initiative for Mothers and Newborns (PAIMAN) Project was launched in 2004. It was a six year project served 24 districts, and two Frontier and two FATA Agencies. Led by JSI Research and Training Institute, Inc. (JSI), PAIMAN was a consortium of international and national partners, including Save the Children US, Population Council, Johns Hopkins University Bloomberg School of Public Health/Center for Communication Programs, Aga Khan University, and Contech International. PAIMAN fully endorsed the National Maternal and Neonatal Health Strategic Framework vision.
Major PAIMAN MNCH strategies were to: reduce maternal, newborn, and child mortality in Pakistan through viable and demonstrable initiatives and capacity-building of existing programs and structures within health systems and communities, to ensure improvements and to strengthen links in the continuum of women's health care from home to hospital. Outcomes were:
•Reduced neonatal mortality rate
•Increased proportion of SBA assisted live births
Study of Sexually Transmitted Infections among Urban Men in Pakistan
Pakistan currently has very low levels of human immunodeficiency virus (HIV)
infection; the overall HIV prevalence rate is less than 0.1 percent in the general
population. However, the country is considered to be at high potential risk for an HIV
epidemic for a number of reasons. In particular, in the major cities there are large
concentrations of individuals engaging in behaviors that make them vulnerable to the
rapid spread of HIV and of classical sexually transmitted infections (STIs).
The following were the objectives identified for the study of urban men:
-Measure the prevalence of selected STIs (gonorrhea, chlamydia, syphilis, genital
herpes [HSV-2], HIV);
-Measure the behaviors that modify the risk of STI, with an emphasis on sexual
behavior and condom use;
-Assess the self-reported STI symptoms and associated treatment-seeking
behavior;
-Measure knowledge of STIs, including HIV, and means of avoiding infection;
-Measure perceived risk of STI/HIV infection; and
-Develop a detailed profile and in-depth characteristics and behavioral patterns
of the bridging population.
