Job Title: Doctor/Health Coordinator

Duty Station: Mardan/CHC

Reporting to: Regional Program Head Technical

Social Capital Development, Education & Literacy, Primary Healthcare, Livelihood and food Security, and Sustainable Environment are key pillars of our program.

Objective: This position is responsible for overseeing all healthcare activity and implementing the HDF Health Program in the Region as per HDF Health Policy.

Key Responsibilities:

a. Health Coordinator (HC) will ensure implementation of all activities /Tasks of the health program at the Community Health Center (CHC) and field are aligned with HDF policies, SOPs, and Standards of quality.

b. Support RPM in preparing DIP of AOP.

c. Ensure smooth implementation of AOP (Health).

d. Examination of outdoor patients at CHC and Mobile Health Units/ Medical Camps.

e. Responsible for bookkeeping/record and data management.

f. Referral of high-risk patients to nearby appropriate health facilities.

g. HC will ensure the

· Daily visits of CHWs.

· Scheduled visits of LHV.

h. Monitoring visits to the program area to assess the performance of LHV and CHWs in the field.

i. HC will visit the field ONCE a week.

j. Implementation of School Health Program.

k. To facilitate LHV in record /data keeping as per standards.
