Job Summary
We are currently seeking dedicated Intake Coordinators to join our team in support of Heal Home Health, a US-based sister company. This full-time position is based in Islamabad, Pakistan, and is well-suited for candidates with 1-2 years of experience in the US healthcare sector. The Intake Coordinator is a critical role responsible for managing patient intake, handling referrals, scheduling, and maintaining patient charts. Serving as the primary liaison among referring providers, patients, clinicians, and administrative personnel, the coordinator ensures a seamless, compliant onboarding process and smooth operations within the home health agency.
Key Responsibilities
The Intake Coordinator will receive and process referrals from a variety of sources including physicians, hospitals, and skilled nursing facilities (SNFs). They will collect and verify essential patient information such as demographics, diagnoses, physician orders, and insurance details. Maintaining accurate and compliant patient charts within the Electronic Medical Record (EMR) system is a key duty, alongside entering data meticulously and managing intake tracking logs.
Insurance verification is another critical task, covering Medicare, Medi-Cal, and HMO/PPO plans, including obtaining necessary authorizations. Coordinating visit schedules with registered nurses (RNs), physical therapists (PTs), occupational therapists (OTs), speech therapists (STs), medical social workers (MSWs), and home health aides (HHAs) is essential to ensure timely patient care.
The coordinator will communicate regularly with physician offices to clarify orders and conduct follow-ups as needed. They are also responsible for auditing patient charts to ensure accuracy, completeness, and compliance with ACHC, CDPH, and CMS standards. Tracking admission dates, start of care (SOC) timelines, reassessments, and missed visits is part of maintaining operational efficiency.
Maintaining HIPAA-compliant records and ensuring secure communication at all times is mandatory. The Intake Coordinator will support the Quality Assurance (QA) team by providing OASIS data, Plan of Care documentation, and ensuring adherence to regulatory timelines. Participation in team meetings and contributing ideas for workflow improvements are encouraged. Additional administrative duties will be performed to support overall agency operations.
Required Qualifications
Candidates must have at least a high school diploma, with an Associate’s or Bachelor’s degree preferred. A minimum of one year of healthcare experience is required, ideally within home health intake. Proficiency in EMR systems and Microsoft Office applications is essential. Strong communication skills, along with excellent multitasking and time management abilities, are necessary for success in this role. A solid understanding of HIPAA regulations, Medicare policies, and payer guidelines is also required.
Preferred Qualifications and Benefits
Familiarity with ACHC and CMS home health standards is highly desirable. Bilingual candidates, particularly those fluent in English and Spanish, will be given preference. The ability to work independently as well as collaboratively within a team environment is important.
This position offers a competitive monthly salary ranging from Rs65,000 to Rs90,000. The role requires in-person attendance at our Islamabad office.
